New Patients
Your First Visit
For Dr. Neubuerger to best diagnose and treat your condition or injury, he needs to have a comprehensive understanding of your current health status and medical history. When you come in for your initial consultation, please bring all the items listed below.
During your visit, Dr. Neubuerger will conduct a comprehensive physical examination and verify your patient history. After your examination and consultation, Dr. Neubuerger will give a detailed explanation of your condition and you will be provided with information about necessary follow-up diagnostics or appointments. Please understand that due to the nature of his practice, emergencies do arise that may cause your appointment to be delayed or even rescheduled.
To Prepare for Your First Appointment:
- Ahead of your appointment, check in online on the patient portal and pay your co-pay. Complete the New Patient Packet.
- Bring CDs of previous X-Rays, CT Scans, or MRI Scans. Films AND Reports must be provided.
- Bring Medical Records of previous testing, treatment, and prior surgeries.
- Prepare and bring a list of all current medications.
- Have your Insurance card on hand.
- Bring Identification: Driver’s License, California ID.
