Dr. Neubuerger is Accepting New Patients

A referral from the treating provider's office is required.

Dr. Neubuerger accepts new patients on a case-by-case basis after reviewing documentation provided by the referring physician’s office.

To ensure that the review process and scheduling of appointments is not delayed, the required documentation must be included and faxed to our office.

Required Referral Documents

Patient Contact Information

Full name, address, phone, and email.

Authorization to Treat

Required for Workman's Comp and HMO patients only.

Imaging Reports (MRI & X-Ray)

Must correspond to the correct body part and be no more than a year old.

Patient Insurance Information

Primary and secondary coverage details.

Last 3 Visit Notes

From the referring provider documenting current condition.

Updated Imaging if Needed

If no current imaging exists, the referring provider must order new imaging.

Please Note:

Lack of up-to-date imaging is the most common reason for delays in scheduling new patient appointments.

The Review Process

Once all medical records, imaging reports and authorizations are received, Dr. Neubuerger personally reviews the information.

STEP 01

Records Received

All required documentation arrives via fax and is logged into our review queue.

STEP 02

Personal Review

Dr. Neubuerger personally reviews each case. The process typically takes 5–7 business days.

STEP 03

Outcome Communicated

If a surgical consult is appropriate, we contact the patient directly to schedule an appointment. If not, we notify the referring provider or adjuster.
